MODULE FIVE-C: FORMS FOR BLOOD TESTING

This module applies ONLY to individuals who have attended and successfully completed an Indiana State Poultry Association NPIP Blood Testing School Class.

INDIANA COUNTY FAIR POULTRY EXHIBITOR FORM

  • This form can be used by the exhibitor to certify that all their poultry to be shown at the county fair came from NPIP P-T Clean flocks, or were P-T tested with 90 days of the start of the county fair

  • Form can be completed by Bird Owner or a certified Blood Tester

  • Bird Owners should always keep a completed copy of the form for their records

  • Superintendents will retain the form in their records

  • It does not need to be submitted to the ISPA office

  • The images below go through how to fill out the form

The top section is completed by the Exhibitor (Bird Owner)

Then, two possible Statements of Fact:

1. The exhibitor certifies the birds are from an official NPIP P-T Clean Flock

  • Note: This certification expires after one year

2. A Blood Tester certifies that the exhibited birds were P-T tested within ninety (90) days of the start of the exhibition

 

USDA APHIS VS FORM 9-2

  • Official USDA Form used by poultry industry to record testing for various diseases

  • Used by the exhibitor to certify that all their poultry to be shown at the county fair were Pullorum Typhoid tested within 90 days

  • Used to certify proper testing done when applying for/renewing NPIP Exhibition Membership in Indiana

  • Form can only be completed by Authorized Agent (Certified Blood Tester)

  • Lastly the signatures

    • "Signature of Inspector or Authorized Agent" is the Blood Tester

    • "Signature of Flock Owner" is the bird owner

  • Where do the different copies go?

    • Part 1 (white copy): Official State Agency Copy – This copy is sent to the ISPA Office

    • Part 2 (blue copy): Agent or Laboratory Copy – This copy stays with the Blood Tester or is submitted to a lab

    • Part 3 (pink copy): Hatcheryman’s Copy – This copy stays with the Blood Tester or given to the Flock Owner

    • Part 4 (yellow copy): Flock Owner’s Copy – This copy goes to the Flock Owner

HOW MANY BIRDS DO I TEST?

  • The answer depends on why you are testing

  • Poultry Show, Exhibition, County Fair, or State Fair

    • ALL Birds that will be attending from an owners flock must be tested for Pullorum Typhoid

  • NPIP Exhibition Membership

    • Test 10% of the flock, with a minimum of 30 birds, up to a maximum of 300 birds between the age 16 – 24 weeks. (For additional details see Blood Testing School Binder)

INDIANA COUNTY FAIR POULTRY SUPERINTENDENT'S NPIP REPORT

  • The Form certifies that all poultry shown at the county fair came from NPIP P-T Clean flocks or were P-T Tested within 90 days of the start of the county fair

  • Form is completed by the Poultry Superintendent

  • Mailed/Faxed/Emailed to the ISPA Office

  • Due within seven days of the close of the County Fair

 

INDIANA COUNTY FAIR POULTRY SUPERINTENDENT'S ADDRESS FORM

  • Completed with the Indiana County Fair Poultry Superintendent's NPIP Report

  • Purpose is to provide the Indiana State Poultry Association with up-to-date contact information for the County Poultry Superintendent
